2.  After you enroll, please call the office (714) 993-9977 to schedule an appointment. You will be required to provide the following documents to complete registration.

• Documentation to verify the name and age of student.
• Immunization Record - Must include complete dates of all immunizations, including diphtheria, pertussis, tetanus, polio, hepatitis B, measles, mumps, rubella, and chickenpox vaccines. Immunization requirements for school entry. Health record form, see below for download.
• Proof of residency along with the completed Residency Verification Form
  
• A current Photo Identification (A government or non-government issued Photo ID)
